Positive social relationships are healthy. A healthy intimate relationship with a life partner can be a source of strength. It's good to talk to people who care about you and listen to you. Close X Mindful Awareness Mindfulness is being fully aware, or paying attention. Sometimes, we go through our daily lives on autopilot. We are not fully aware of the present. We often dwell on the past and plan events in the future. We do not spend much time really paying attention and noticing what is happening right now; without judging or trying to fix it. Your body and mind send you signals constantly. If your attention is elsewhere, you don't notice. Then, the signals that began as whispers become loud warnings. Being mindful, or aware, allows you to make conscious proactive choices about every aspect of your health. Mindfulness connects you to each component of your well-being. Close X Community The outer ring represents your community. For some, their community is close and for others it is far away. Your community is more than the places where you live, work, and worship. It includes all the people and groups you connect with; who rely on you and upon whom you rely. Close X Professional Care Prevention and treatment of illness or disease and traditional and complementary medicine are part of professional care. Preventive care includes things like immunizations and cancer screening. Common treatments include check-ups, medicines, supplements, physical therapy, surgery, and counseling. Our Veterans will be taught kidney prevention and improvement objectives which include: priorities, anatomy and physiology, pathophysiology, stages of kidney disease, reading kidney focused lab reports, renal nourishment, building a kidney balanced meal, kidney focused nutrition fact label reading, kidney focused grocery shopping, kidney hydration, kidney movement, tracking prevention, SMART goal setting, and self-reflection. GeroFit

Gerofit is an exercise program that promotes health & wellness for Veterans 65 years and older. Participants in the program have demonstrated improved health, mental, physical function and overall well-being. Gerofit is a great way for Veterans to stay active. Groups focus on Tai Chi, balance training, cardiorespiratory fitness, UE/LE strength training, flexibility and stretching.

MOVE! is a weight management, health promotion program designed to improve the lives of Veterans. MOVE!'s core ideas (encouraging healthy eating behavior, increasing physical activity, and promoting even small weight losses) are easy to follow and based on the latest in nutrition science. With the help of your MOVE! care team, you can reduce health risks, prevent or reverse certain diseases, improve your quality of life, and even live longer! We now offer a women's-only group to discuss women-specific needs and concerns regarding weight management and overall health. Virtual, 1 time class

MOVE! 14 week series

MOVE! is a weight management program designed to improve the lives of Veterans. MOVE!'s core components are behavioral strategies, nutrition changes, and increased physical activity. These are combined to promote even small weight losses and are easy to follow. The program is based on the latest in nutrition science. Virtual, 14 sessions. This program does qualify for the Comprehensive Lifestyle Intervention required for medication eligibility.

Nutrition and Prevention (Wellness-1 time Class)

Welcome to our VA Wellness Prevention Nutrition class. This class will happen two times per month and the veteran can attend anytime. Our Veterans will be taught to learn lifestyle management focused on living well. The class is dedicated to prioritizing mental and physical health by incorporating three foundation pieces of health. The veterans will be guided to focus on mental health first by choosing their life priorities and joys, learning to practice gratitude and mindfulness, and developing a sleep routine. The foundation pieces of health will be centered on hydrating, eating, and moving well. Virtual, 1 time class

Our Veterans will be taught Pre-DM and DM prevention and improvement objectives which include: priorities, anatomy and physiology, pathophysiology, differences in DM (Pre- DM, DM1, and DM2), reading DM lab reports, hydration, DM nourishment steps in building a balanced plate, portion size visuals and recommendations, DM focused nutrition fact label reading and grocery shopping, carbohydrate counting, insulin basics, hypo and hyper glycemic symptoms, treatment steps for hypoglycemia, sleep routine development, SMART goal setting, tracking intervention progress, and self-reflection in health. Tobacco and Vaping Use Treatment

This virtual tobacco use and vaping use treatment program looks at values, provides an overview of medication options and behavioral strategies to support Veterans in their decision to quit using tobacco. Veterans are able to leave with quit and prescription plan, as appropriate. One-on-one health coaching is available with the program! Virtual, 1 time class. Weight Management Medications 101

The Weight Management Medications 101 class is designed to give an in-depth overview of available weight management medications. The MOVE Physician will discuss risks and benefits, expectations, and who are good candidates. For those interested in being prescribed weight loss medications, please reach out to your PACT. Medications require Comprehensive Lifestyle Intervention participation in the last year.
